Fertility and testicular cancer: histological type and seminogram relation

Se ha demostrado que el recuento de espermatozoides móviles tiende a disminuir a medida que aumenta el estadio del cáncer en los pacientes que depositan su semen en un banco de semen. En un estudio, la concentración media de espermatozoides de los pacientes con seminomas fue de 50 millones/mL, mientras que los enfermos con Tumor de Células Germinales No Seminomatoso (TCGNS) la media descendió a 17 millones/mL. Objetivo: Evaluar la relación que existe entre la presentación histopatológica y los resultados del seminograma en los pacientes operados de orquiectomía radical en el servicio de Urología del Hospital Juárez de México. Métodos: De marzo 2006 a mayo 2007, se realizó un estudio clínico, prospectivo, longitudinal y original; llevándose a cabo en pacientes con tumor testicular en el Servicio de Urología del Hospital Juárez de México, en quienes se efectuó orquiectomía radical. Se realizó espermatobioscopia directa preoperatoria a todos los pacientes y un análisis estadístico de los resultados en relación con la estirpe del tumor, tiempo de evolución y estadio. Resultados: Se realizaron 22 orquiectomías radicales, 18 pacientes presentaron alteraciones en la espermatobioscopia, 6 con una alteración y 12 con 2 o más alteraciones, predominando necrospermia, hipospermia y astenospermia. Conclusiones: Existe relación entre el cáncer testicular y las alteraciones en el seminograma; la estirpe histológica, el tiempo de evolución y el estadio del tumor se relacionan directamente con alteraciones específicas en los resultados del mismo.
